Telehealth Visits: A Letter to Our Patients

Dear Patients of Northwest Colorado Health,

We are dedicated to providing high quality, affordable care for anyone who needs it in Northwest Colorado. In light of the rapidly evolving COVID-19 pandemic, we want you to know we are here for you. Effective March 23, 2020 we will be seeing all patients virtually rather than in clinic, unless it’s determined as medically necessary by the provider. This means that you can access your Northwest Colorado Health medical or behavioral health provider via video chat and/or phone consultations. This new mode of care is called “telehealth” and allows you to see and speak with your provider from the comfort of your own home. We ask that all patients call ahead to schedule any appointments instead of coming directly to the Community Health Center.

To make a telehealth appointment with your medical or behavioral health provider, call 970-879-1632 in Steamboat Springs or 970-824-8233 in Craig. You, as a patient, will need access to email and a phone or computer (enabled with a video camera, microphone and speaker) if doing a video visit. At the time of your appointment, the provider’s Assistant will call you to check in and complete initial paperwork. Then you will connect on your computer or phone with your medical or mental health provider. If any technical difficulties arise during a video chat, phone consults will be used instead.

Additionally, all patients have access to communicate with your medical team about non-urgent needs through our patient portal – myClinicOnline. Once you login, you can request medication refill requests, send messages to your provider, view lab results and more.

If telehealth is not an option for you/your needs, please contact us to schedule a phone consultation as needed. We are still open and accepting medical patients if an in-person visit is needed. We are happy to be able to provide these options for you to ensure your safety and reduce your risk of COVID transmission.
