Northwest Colorado Health’s Community Health Center Recognized as Quality Leader

Northwest Colorado Health has received the Health Center Quality Leader (Bronze) award for providing high quality care resulting in improved health for patients at clinics in Moffat and Routt Counties. This award places Northwest Colorado Health in the top 30 percent of health centers in the nation for service and quality achievements. The U.S. Department of Health and Human Services (HHS) announced the Quality Improvement Awards to 1,087 health centers across the U.S. Funded by the Health Resources and Services Administration (HRSA), health centers will use these awards to improve the quality, efficiency, and value of the health care they provide. Northwest Colorado Health also received Community Health Quality Recognition badges for Addressing Social Risk Factors to Health and Advancing Health Information Technology. After the most recent operational site visit, HRSA commended Northwest Colorado Health for being a top performer in quality and compliance as a Federally Qualified Health Center.

The organization has also earned accreditation as a Patient Centered Medical Home by the National Committee for Quality Assurance. The designation is achieved by demonstrating a focus on patient centered care, which seeks to strengthen relationships between patients and their clinical team to improve patient experience and health outcomes while lowering health care costs.
