Northwest Colorado Health is pleased to announce that Sally Habermehl has taken the position of Director of Home Services. Sally has been a valuable team member of Northwest Colorado Health’s Home Health and Hospice program for more than 25 years as a Physical Therapist. Sally earned her Bachelor of Science degree from the College of Saint Scholastic in 1989. She worked as a Physical Therapist in a variety of settings before joining Northwest Colorado Health in 1997 as a Home Health Physical Therapist. She was the Therapy Team Lead for many years and transitioned to interim Director of Home Services in the summer of 2023, then was hired as the Director of Home Services in July 2023. The Director of Home Services plans, directs, and supervises the Home Health, Hospice and HCBS programs. In this position, Sally is responsible for the overall quality of patient care and collaboration with community partners. Northwest Colorado Health’s Home Services program is taking referrals for all services in Moffat and Routt counties.
